The layers of the Smell were complex and hard to describe, like the odor resulting from mixing rotten, moldy raw meat thrown into a trash can full of vomit and swill, then left to ferment under the Sun for three days and nights.
Just accidentally inhaling a little made Wu Chang dizzy; he scrambled back to the first floor and only dared to return underground after putting on a heavy medical mask.
The layout of the Underground first floor was similar to the wards above ground—the same corridors, the same Room layout—the only difference was that the Iron doors were heavier and could only be opened from the outside.
Looking inside through the Iron door's fence, one could see the patients in the severe illness wards, all fixed onto beds equipped with pulleys; they were wrapped in layers of leather straitjackets, which were further secured with iron chains.
Even so, they were constantly struggling, trying to break free from their restraints.
Some had even succeeded.
The patient in Severe Illness Ward Number Three had a fist-sized hole torn in the leather straitjacket on their side abdomen.
At first, Wu Chang thought the patient had clawed through the straitjacket from the inside, but upon closer inspection, he realized the "fingers" protruding from the tear were fleshy tentacles, stripped of their skin.
They were gnawing on each other with the small mouths at their tips, using the oozing Blood to Corrode the leather straitjacket.

Further inside the severe illness wards was the Sanatorium's morgue.
Patients who died from Delusion Syndrome were stored there; they maintained their restrained posture, fixed to the walls on both sides with iron chains, awaiting the end of everything before being sent to the Cemetery for Burial.
Following the scene he saw in the Grudge, he found a Secret Tunnel at the end of the Underground first floor, and beyond the Secret Tunnel was the pure White Iron Door that had Appeared in the Deep Red Grudge.
Staring at the White Door before him, he recalled the Deep Red Grudge he had seen on Jinton.
The Master of Resentment this time was stronger than the Mage and the Gambler, both in overall raw strength and in the ability to execute Instance Strategy.

Jinton was cautious in his movements and not weak in strength, making a direct kill difficult; the Doctor planned to use Jinton's method—winning the key through a gamble.
Unfortunately, all the Players who gambled with Jinton died, and no one brought the Intelligence out of the Instance, causing countless Players to repeat the same mistake on Jinton.
In the Doctor's eyes, a mere Instance Elite Monster could be easily handled with a flick of his fingers.
But he didn't know that Jinton at the gambling table possessed some kind of Special cheating Ability; even the Gambler had capsized in the gutter, and the Doctor, who was not skilled at gambling, naturally fell for it, dying inexplicably at the gambling table.
